Quail's finding, and their isolation of it is what made it one line.

caught    "A correction has been filed for review."
SURVIVES  "An issue has already been filed for this."

passiveActionClaim required has and been to be adjacent. Same claim, one adverb apart.

already was never the problem. Adjacency was. That distinction is why this is not another word-list round — it is the fifth widening of these patterns today and the first whose cause is structural rather than lexical.

Bounded, not open

The adverb slot is (?:already |just |now |recently |since )? rather than \w+\s+.

An open gap would carry not past the auxiliary, so "An issue has not been filed" would reach passiveActionClaim and survive only because notAClaim catches it separately. That turns one dependency into two, and it is exactly the shape of next week's regression.

Measured, both directions

An issue has already been filed for this.        caught
A correction has just been filed.                caught
A tracking issue has recently been created.      caught
A correction has been filed for review.          caught     <- unchanged
No issue has been filed for this.                SURVIVES
An issue has not been filed for this.            SURVIVES
No correction has already been filed.            SURVIVES   <- the denial, through the new slot
A correction has been filed by another member.   SURVIVES

Four rows into the corpus: three catches and the denial, because the denial is the half a widened auxiliary threatens and the one worth pinning.

Worth saying

This is the fourth of these I have made today, each correct and each incomplete. The word lists are carrying more than word lists can. I am not proposing a rewrite in the middle of a queue of small correct fixes, but somebody should decide whether this stays a regex — the evidence is now five rounds deep and in the thread.

Quail isolated it after the pastReference round, which is what made it a one
line change rather than another word list. already was never the problem.
Adjacency was.

The adverb slot is bounded rather than open. A \w+ gap would carry not past the
auxiliary and leave the denial to notAClaim alone, which is a second dependency
where there is currently one, and the shape of next week's regression.

Four rows go in the corpus, three catches and one denial, because the denial is
the half a widened auxiliary threatens.
