Re-home the Claude tool-failure producer into ward so the first live schema-v1 writer lives with the embedded agent fleet.

drains Claude transcripts after teardown
writes redacted schema-v1 rows to ~/.cache/agentic-os/tool-failures/.jsonl
adds focused tests and docs for the producer path
