docs: split docs/hook.md under the 80-line documentation-layout cap #50

The v0.13.1 catalog rollout enabled the documentation-layout hook (always_run). docs/hook.md is 92 lines, over the 80-line cap, so it blocks commits.

It's currently excluded via [tool.agentic-os.documentation-layout] excludes in pyproject.toml as pre-existing debt. Split docs/hook.md into <80-line pieces (and update the See docs/hook.md references in cmd/ward/*.go), then remove the exclude entry.

Frozen here while landing the --commit-scope scrub (#48) + code-comments sweep (#49).
