docs: consolidate warp/README.md into docs/, drop the subsystem README

Following docs/warp.md (the Go-module model), fold the host-facing content of warp/README.md into docs/ and remove the co-located subsystem README so Warp docs live in one place under the cap regime.

Add docs/warp-host-setup.md (channels, install playbook, fallback, subdirs) - the README content, table converted to flat bullets per voice rules, trimmed under the 80-line/4000-char docs cap.
Delete warp/README.md and drop its [tool.agentic-os.documentation-layout] excludes entry (the file no longer needs the #22 escape hatch).
Repoint every reference: docs/warp.md, root README.md, tooling-warp SKILL.md + references/channels.md.

Out of scope: warp/launch_configurations/README.md (a deeper profile-design reference, still covered by the launch_configurations/** exclude).
